BACKROUND
Karisma Cimareme Hospital in West Bandung Regency in Indonesia is a private hospital owned by Karisma Aradi Healthcare. The company was established in 2010 solely as a platform to build and operate the Hospital as required by Indonesian law. The founder was Yedi Suyadi, a local physician interested to provide low cost, better quality healthcare services in West Bandung.
Currently Indonesia is still riddled with a lot of healthcare problems. Maternal and Child mortality rate is among the highest in the Asia. Infectious disease long eradicated in the developed world such as Tuberculosis are still a common findings here. Now even cardiometabolic diseases are on the rise. Although Indonesia has enormous healthcare problem, the national healthcare budget at only 3.5% of our national budget is still very low compared to other country. This low budget considerably effect the availability of healthcare facility in Indonesia. As an illustration, although heart disease is the number one killer in West Java province that has approximately 46 million inhabitants there's only one hospital with primary PCI capability. Primary PCI is very helpful for a patient with major heart attack. Without it, heart muscle will dies and the patient can experience heart failure or even sudden death.

CURRENT SITUATION AND DETAILED NEEDS
Kabupaten Bandung Barat or West Bandung Regency is located in West Java Province in Indonesia. According to the 2008 census the regency has a population of 1,531,072 inhabitants. Currently the people in West Bandung Regency have access to 3 General Hospital in the area namely Cahya Kawaluyan Hospital (RSCK) with approximately 90 beds, Indra Hospital Medical Center (IMC) with 60 beds, and Hospital Karisma Cimareme (RSKC) with 74 beds. The local government currently in the process of developing two technical Implementation Units in Cililin and Lembang into a small hospital. Among the three hospitals, Intensive Care is only available in RSCK with 3 beds for intermediate care, 2 bed for Pediatric Intensive Care Unit (PICU), and 2 bed for Intensive Care Unit (ICU). Because the limited bed capacity people in West Bandung Regency who is severely ill often referred to faraway hospital in Bandung or Cimahi. However intensive care facilities in many hospitals in Bandung or Cimahi are currently almost always full with long waiting list. This is very worrying and needs every attention from everybody concerned.
Karisma Cimareme was originally a clinic named Klinik Cimareme (KC) that was built in the early 90s. It’s located in the regency district capital Ngamprah next to the main road and a crossroad that connect all the other districts in West Bandung Regency. In 2010 we decided to expand our services and transformed into a hospital named Rumah Sakit Karisma Cimareme (RSKC). Karisma Cimareme main hospital building construction was started in late 2010 and completed in mid 2014. We now have a 5 storey main building that provides affordable consultations with physicians and specialists, blood diagnostic testing, access to medications, and free education and awareness programs. Our hospital currently has 3 wards with 74 beds for hospitalization supported by 20 nurses, 4 midwife, 6 general physician, and 13 specialist.

We currently have 6 general physician and 13 specialist practicing Medicine in RSKC. Our specialist includes dr. Yedi Suyadi SpPD and dr. Eddy Harjadi SpPD as our internal medicine specialist, dr. Erta Priadi W. SpJP as our cardiologist, Prof. DR. Deddy Rachmadi SpA and dr. Melani Sari SpA as our paediatrician, dr. Zulkifli Said SpOG and dr. Evi Arijani SpOG as our obstetrician, dr. Nanda Dewi SpKK as our dermatologist, dr. Agung D. Permana SpTHT as our ENT specialist, dr. Erna Tjahyaningtyas SpM as our Ophthalmologist, dr. Maria Octaviany SpS as our Neurologist, dr. Rudy Silaen SpB as our general surgeon, and dr. Immanuel Wiraatmadja SpAn as our Anaesthesiologist. We’ve also have an agreement with subspecialist such as dr. Dali Hanafiah SpBO an orthopaedic in Hasan Sadikin Central Hospital, dr. Irfan SpBU an urologist in Cibabat General Hospital, and dr. Lukmana Lokardjana SpBU, KBD an expert in digestive tract surgery in Dustira Army Hospital so that they can handle complex surgery in our hospital.
We have prepared 2 major operating rooms with a space large enough to handle complex surgeries such as severe traumas, tumorous cancer removals, major digestive tract surgeries, and even complex cardiac procedures. However due to limited funding we can only equipped one operating theatre with equipment that only allow simple procedure. We’ve also prepared shielded room for CT-scan and also cardiac catheterisations so that in the future we can handle primary PCI, endoscopic/laparoscopic treatment, and a dialysis center.
Karisma Cimareme Hospital therefore, have the required space and recruited the necessary personnel to allow us to treat more patient and handle more complex cases. However before the hospital can become fully operational, it is imperative that we procure more medical equipment that will allow us to serve and saves even more people. Trough donors help we are hoping to secure the needed funds / medical devices, pushed the incurred medical cost to our patient lower and allow even high quality, normally high cost medical treatment or surgeries to those in need. In the end it is our hope that in the future we can help the local government solve the community healthcare problems by providing high quality healthcare that is affordable and accessible to all without the need for referral to other, faraway centers.

We are currently seeking funding to buy medical devices or medical devices donation that would allow us to expand our current diagnostic laboratory so we can better diagnose our patient, allow treatment of severely ill baby, children or adult in our intensive care unit and perform more complex, life-saving surgical operations to our patients who attend the Karisma Cimareme Hospital.
In order to save more lives we would need approximately 3,4 million dollars to fully equipped our hospital. We however have prioritize our needs and we urgently needs to expands our diagnostic capability, allow intensive care treatment, fully equipped our operating theatre, and open the rehabilitation unit. To do all that we need approximately 1.26 million dollars, the list of items needed and their estimated cost can be seen below.

PROJECTED COST
Many different pieces of medical equipment are required to better serve the people in West Bandung. These items, in addition to their costs, are described below in three separate sections based on their priority or development phase.
First Phase Procurement
We currently have basic laboratory equipment but our ageing equipment need replacement as soon as possible. As a hospital we need more laboratory equipment to handle more patient volumes. We also have skilled doctors who can do echocardiography or endoscopic procedure but we haven’t got the devices needed to do perform the procedure.
We already have a running major operating room, but we need better anesthesia machine equipped with ventilator to handle more prolonged & complex surgeries. We also need surgical ophtalmoscopy microscope so that our ophthalmologist can do delicate eye surgery. As we mention before, we already have the room that we can use as an intensive care unit, but we still need ICU beds, incubators, vital sign monitors, portable x-ray, defibrillators, infusion & syringe pumps so that we can open the intensive care unit. This in conjunction with fully equipped operating theatre, allow us to treat high risk patient that need major surgery or patient that just had complex major surgery with unstable condition post operatively.
Patients who just had an accident, heart attack, or stroke are often discharged without undergoing rehabilitation process because there’s no medical rehabilitation unit. We consider medical rehabilitation as an integral part of patient therapy so we consider it as one of our highest priority to insure our patient survival and better quality of life.

The fourth floor in our hospital consist of Intensive Care Unit, 2 Major and 1 Minor Operating Room, Dialysis Center, and Endoscopy Exam. Currently due to limited funding, only one operating room that's open.

We also need to purchase CT-scan that allow as to diagnose the existence of deep tissue mass, assess traumatic injury, reason behind altered consciousness, existence coronary artery disease, etc. With availability of CT-scan, we doesn't have to refer patient to other hospital for diagnostic reason, this will not only save our patients time but also allow us to quickly diagnose our patient and rapidly give them appropriate treatment. Right now there’s no hospital with dialysis center or dialysis clinic in West Bandung. We aims to open our dialysis center so that patient with kidney failure doesn't have to go to faraway to have their routine dialysis.

This is our secondary operating room. If we can provide the operating room, lamp, anesthesia machine, vital sign monitor, defibrillator, etc. We can do more surgery in our hospital and safe more life's.

This is our planned dialysis room. In west Bandung we still doesn't have a dialysis center, So the people here have to go to faraway center for routine dialysis. If we can provide the facility it would mean a lot for a lot of patient that has chronic kidney failures here. We've provided the rooms with enough sunlight, with good AC and TV so that they can enjoy their time during dialysis.

Third Phase Procurement
As I said earlier currently West Java Province has 46 million inhabitants, however we only have one center with Primary PCI capability. We already have a shielded room that’s large enough to allow installation of hybrid Cath lab. This will allow us to do diagnostic coronary angiography, per-cutaneous coronary intervention, emergency cardiac intervention (Primary / Urgent PCI), placement pacemaker or other cardiac devices for people with heart disease, and even targeted chemotherapy. To insure the availability of interventional cardiologist we plan to cooperate with with Padjajaran University Cardiovascular Department. It is our dream that someday RSKC can become a center for cardiac intervention that can be useful to the people of West Java. |
This is our lead shielded room. We have prepared it from now because we wanted our hospital be an excellent cardiac center in the future. The device however is very costly and it is unlikely we can purchase it if we do it ourselves.

IMPACT TO THE COMMUNITY
It is our believes that access to medical services should be affordable to all. West Bandung Regency has large geographic and population size but have limited health facilities provided to them. Currently, people in West Bandung who require advance diagnostic testing, hospitalisation in the intensive care, and complex surgical/interventional procedures must travel long distances to receive these services; at prohibitively expensive costs and often the unavailability of beds.Procuring the medical and hospital equipment listed above will allow us to provide high quality healthcare and pushed the overall costs of healthcare services we provide. This will have an enormous impact on the people health not only in West Bandung but also in West Java.
WHAT WILL THE DONORS GET?
As we said earlier we won't charge the patient for the use of the donated medical devices, we do however still need to charge the patient for doctors, nurse, electricity, and administration cost. This in our estimation would lower the overall cost in our hospital by halves if nor more. So people that donate the needed medical equipments is like subsidising our patient treatment for years to come, as long as we can still use the medical devices.
We realize that the total funds needed will be difficult to be met by a single donor. Therefore we are ready to cooperate with various parties in order to achieve our goal of providing high quality health care affordable and accessible to all.

Other contribution such as:
- If you directly donate the medical device we need, we'll install a plaque with the donor on the medical devices. If the devices cost more than 10.000$ we'll put your name in the charter. If the donated devices allows us to produce an exam report, your name / the donating company name will be mention as the contributor in the examination report.
- If you donate the entirety of necessary funds or medical devices that enable us to open facilities such as ICU, intermediate care room, operating room, cardiac catheterisation labs or dialysis center we'll use you name as the room name. You'll be invited for the facility opening ceremony and a vacation in Bandung.
